|Shaler Area School District contracts with UPMC Sports Medicine to provide Certified Athletic Trainers for PIAA sanctioned athletics in the district. The Athletic Trainers are based at the High School, Middle School and the Elementary School. Work locations and covered sports vary with the seasons.

For ALL SPORTS the certification of physical date on the PIAA Physical Form Section 4 must be no earlier than June 1, 2017. All physicals regardless of when taken for a current school year will expire on May 31st of that school year.
We only accept this packet for athletics!
The PIAA requires one comprehensive physical per year for eligibility to participate in interscholastic athletics. The PIAA requires that the date on the PIAA Physicians Form Section 6 must be no earlier than June 1, 2017 for the upcoming school year. If the date on the form is incorrect the physical will be returned and the athlete will be ineligible to participate until the physical is dated properly. We do not require any immunization information as is stated in the physical packet. We will not accept any physical paperwork that is not on the PIAA AND Shaler Area forms that are downloadable at the bottom of this page.
The fee for all physicals taken at school is $10.00 and is due at the time of the Physical.
ALL PHYSICAL FORMS MUST BE FILLED OUT COMPLETELY AND SIGNED BY A PARENT OR GUARDIAN TO RECEIVE A PHYSICAL.

SPRING SPORTS PHYSICALS FOR ALL STUDENTS (GRADES 7 – 12) – WILL BE HELD ON:
Tuesday, February 13, 2018 at the High School beginning at 2:30 pm.
Report to Room 214 of the High School after school. All forms must be filled out and signed by a parent / guardian to receive a physical. A shuttle bus will be provided from the middle school.
Physicals are given on a first come first served basis. – No exceptions.
