The PIAA requires one comprehensive physical per year for eligibility to participate in interscholastic athletics. The PIAA requires that the date on the PIAA Physicians Form Section 6 must be no earlier than June 1, 2019 for the upcoming school year. If the date on the form is incorrect the physical will be returned and the athlete will be ineligible to participate until the physical is dated properly. We do not require any immunization information as is stated in the physical packet. We will not accept any physical paperwork that is not on the PIAA AND Shaler Area forms that are downloadable at the bottom of this page.
The fee for all physicals taken at school is $10.00 and is due at the time of the Physical. Along with the physical, a Functional Movement Assessment will also be completed for all attending student-athletes, FREE OF CHARGE!
Checks are preferred and can be made out to “S.A.S.D.”
ALL PHYSICAL FORMS MUST BE FILLED OUT COMPLETELY AND SIGNED BY A PARENT OR GUARDIAN TO RECEIVE A PHYSICAL.
