Fall Sports Physicals

SPORTS PHYSICALS

The PIAA requires one comprehensive physical per year for eligibility to participate in interscholastic athletics. The PIAA requires that the date on the PIAA Physicians Form Section 6 must be no earlier than June 1, 2020 for the upcoming school year.  If the date on the form is incorrect the physical will be returned and the athlete will be ineligible to participate until the physical is dated properly.  We do not require any immunization information as is stated in the physical packet.  We will not accept any physical paperwork that is not on the PIAA AND SHALER AREA forms that are downloadable at the bottom of this page.

The fee for all physicals taken at school is $10.00 and is due at the time of the Physical. There will NOT be a Functional Movement assessment offered for fall sports physicals.

Checks are preferred and can be made out to “S.A.S.D.”

ALL PHYSICAL FORMS MUST BE FILLED OUT COMPLETELY AND SIGNED BY A PARENT OR GUARDIAN TO RECEIVE A PHYSICAL. WE ARE ASKING THAT ONLY FALL SPORTS REPORT FOR PHYSICALS AT THE DESIGNATED TIMES. WE WILL BE PROVIDING PHYSICALS FOR WINTER AND SPRING SPORTS AT A LATER DATE.

 

FALL SPORTS PHYSICALS FOR ALL STUDENTS (GRADES 7 – 12) – WILL BE HELD ON: 8/3/2020 and 8/4/2020 at the high school. Your report time is based on the first letter of your last name, so we don’t have people waiting for long periods of time and we are practicing social distancing. You will not be permitted in the school before or after your scheduled time. A mask will be required to enter the building. See schedule below:

All forms must be filled out and signed by a parent / guardian and payment is required to receive a physical.

 

Someone will meet you at the front door of the high school, you may be asked a few questions to clear you from any COVID-19 symptoms and you will be escorted back to where the physicals are taking place. Again, you must have a mask to cover your mouth and nose and please do not report before your scheduled time.  If you have any questions or issues please email the sports medicine staff at athletictraining@sasd.k12.pa.us
